Your First Visit

We are excited to meet you and feel honored that you have chosen our practice for your dental needs. We will work to earn your trust and hope you will feel comfortable referring your friends and family here.

To help your first visit run smoothly please print off the attached Medical History and Patient Registration form and fill them out. Please arrive 10 minutes early to provide adequate time to confirm medical history and insurance information as well. If you prefer to wait to fill out your forms until you get here, please allow adequate time for this in our office before your appointment.

Your first visit will be a time for you to meet the doctor and communicate your dental goals and needs. With your permission we will gather the information we need to make recommendations that are appropriate for you. This time may include a review of your health history, intra- and extra-oral exam, oral cancer screening, dental and periodontal charting, any necessary x-rays and consultation with the doctor.

We ask that if your appointment needs to be canceled or rescheduled that you do so at least 48 hours prior to your appointment.
